Brief Description: This product supports patients in self-management at home through symptom monitoring, goal setting, educational tools, secure messaging and video visits. It connects patients with their complete circle of care including health providers, family members and community supports.

Art-on-the-Brain

Key Objective: Address the problem of reduced access to meaningful recreation among older adults with complex health conditions.

Brief Description: Art-on-the-Brain is an affordable online recreation activity designed for all older adults including those with cognitive decline. This scientifically validated intervention encourages socialization and builds relationships between the user and their circle of care, using visual art presented in an interactive online environment to stimulate cognition and encourage social connections amongst users through a series of enjoyable learning and gaming activities.

Brief Description: Deciding to start or continue an antidepressant medication during pregnancy can be a difficult and complex decision. This online interactive patient decision tool can help women make choices about antidepressant use in pregnancy.

ePRO

Key Objective: Help providers and individuals better manage complex care needs.

Brief Description: The electronic Patient Reported Outcome (ePRO) tool is designed to assist individuals with complex care needs and their primary care providers develop and monitor SMART (Specific, Measurable, Achievable, Relevant, Time-Based) goals and health outcomes. The ePRO tool is available on two platforms: a mobile app available on smartphones and an online desktop portal.

Key Objective: To prepare University of Toronto students, staff and faculty to identify, assist and refer students experiencing mental health challenges to services and resources.

Brief Description: When we refer to mental health, we’re describing the capacity of each and every one of us to feel, think, and act in ways that enhance our ability to enjoy life and deal with the everyday challenges we face. Our mental health exists along a continuum from flourishing to languishing, and is separate and distinct from mental illness. This training module gives students, staff and faculty tools to help them identify, assist and refer students experiencing mental health challenges.

Key Objective: An electronic decision support tool for managing asthma and improving the quality of asthma care in primary care settings.

Brief Description: Electronic Asthma Management System (eAMS) is an evidence-based computerized decision support tool. It was designed to help providers to bridge the 3 main evidence-to-practice gaps that have been identified across care settings and are believed to be the major cause of poor disease control and healthcare utilization in patients with asthma.

Target Audience: Primary care providers.

KeepWell

Lead: Dr. Monika Kastner, North York General Hospital

Key Objective: Optimizing the health and wellness of older adults with multiple chronic conditions.

Brief Description: KeepWell is a web-based knowledge translation tool built to optimize self-management of older adults and to promote communication with providers to facilitate shared decision making about optimized multimorbidity management. KeepWell integrates the care of any combination of the 10 most common high-burden chronic conditions affecting older adults (e.g., Diabetes, Heart failure, Dementia, Arthritis, Depression), responds to the complexities of disease concordance/discordance in multimorbidity management, and has the potential to provide clinical decision support for providers.

Target Audience: Older adults (65+) at risk for or living with multiple chronic conditions and their primary care providers.

A 6-month pragmatic randomized controlled trial (RCT) to evaluate KeepWell with 400+ older adults will be launched in the fall of 2020.
